Guidelines governing your use of Treatment Vital services
Welcome to Treatment Vital. These Terms & Conditions ("Terms") govern your access to and use of our website, mobile application, and wellness services ("Services"). By booking a consultation or using our Services, you agree to be bound by these Terms. Please read them carefully.
By accessing our website, scheduling an appointment, or engaging any Treatment Vital service, you confirm that you are at least 18 years old (or have parental/guardian authorization if a minor), have legal capacity to enter binding agreements, and agree to these Terms, our Privacy Policy, and any supplemental documents referenced herein.
Treatment Vital provides concierge-style home health and wellness management, including but not limited to general healthcare consultations, dental care, nutrition and weight loss coaching, hair and skin treatments, joint and bone health services, cardiovascular monitoring, gender-specific healthcare, memory and neurological care, and health insurance navigation. Services are delivered by licensed, credentialed professionals either in-person at your home, via telehealth, or through our digital platform.
Certain features require an account. You agree to provide accurate, current, and complete information, and to maintain the security of your login credentials. You are responsible for all activity under your account.
Our Signature and Elite packages operate on a monthly subscription basis, billed automatically on the same date each month. The Essential package may be purchased monthly or as a one-time engagement.
Prices are listed in U.S. dollars and may change with 30 days' written notice. Promotional pricing is valid only for the period advertised.
We accept major credit/debit cards, ACH transfers, and HSA/FSA cards. All payments are processed through PCI DSS-compliant payment processors.
If a payment fails, we will retry and may suspend services after three unsuccessful attempts. A $25 late fee applies after 15 days past due.
You may cancel any monthly subscription with 30 days' written notice. Cancellation takes effect at the end of your current billing cycle.
Individual appointments may be cancelled or rescheduled at no charge with 24 hours' notice. Cancellations within 24 hours incur a $75 fee. No-shows are charged the full session rate.
We offer a 14-day satisfaction guarantee on first-time memberships. If you are dissatisfied within the first 14 days, we will refund your initial payment minus the cost of any services already rendered. After 14 days, unused portions of prepaid packages are non-refundable but may be applied toward future services within 12 months.
Treatment Vital provides wellness, preventive, and lifestyle-focused services. While our practitioners are licensed healthcare professionals, our Services do not replace emergency medical care, acute hospitalization, or specialty surgical treatment. In any medical emergency, call 911 or go to your nearest emergency room immediately. Our practitioners do not diagnose or treat serious, life-threatening conditions outside their scope of practice.
Before any clinical service, you will receive and sign an informed consent document explaining the nature, risks, benefits, and alternatives of the treatment. You have the right to refuse any procedure at any time.

All content on our website and mobile app — including logos, text, images, videos, software, and proprietary wellness protocols — is owned by Treatment Vital or its licensors and protected by U.S. and international copyright, trademark, and trade secret laws. You may not reproduce, distribute, or create derivative works without our written permission.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, Treatment Vital, ITS AFFILIATES, OFFICERS, EMPLOYEES, AND INDEPENDENT PRACTITIONERS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM OR RELATED TO YOUR USE OF OUR SERVICES. TOTAL AGGREGATE LIABILITY SHALL NOT EXCEED THE AMOUNTS YOU PAID US IN THE TWELVE (12) MONTHS PRECEDING THE CLAIM. THIS LIMITATION APPLIES REGARDLESS OF THE LEGAL THEORY ON WHICH THE CLAIM IS BASED.
You agree to indemnify, defend, and hold harmless Treatment Vital and its representatives from any claims, losses, damages, or expenses (including reasonable attorneys' fees) arising from your breach of these Terms, your violation of any law, or your misuse of our Services.
Before initiating formal proceedings, you agree to contact us at legal@treatmentvital.com and attempt to resolve any dispute informally for 30 days.
Any dispute that cannot be resolved informally shall be settled by binding arbitration administered by the American Arbitration Association under its Commercial Arbitration Rules, conducted in Louisville, Kentucky.
You agree to resolve disputes on an individual basis only and waive any right to participate in a class, consolidated, or representative action.
These Terms are governed by the laws of the Commonwealth of Kentucky, without regard to conflict-of-law principles. Any non-arbitrable claims shall be brought exclusively in the state or federal courts located in Jefferson County, Kentucky.
We may suspend or terminate your account or services, with or without notice, for any breach of these Terms, fraudulent behavior, or non-payment. Upon termination, your right to use our Services ceases immediately, but provisions that by their nature should survive (e.g., payment obligations, indemnification, dispute resolution) remain in effect.
We may update these Terms at any time. Material changes will be communicated via email or website notice at least 30 days in advance. Continued use of Services after changes take effect constitutes acceptance.
If any provision is found unenforceable, the remaining provisions remain in full force and effect.
These Terms, together with our Privacy Policy and any signed informed consent documents, constitute the entire agreement between you and Treatment Vital regarding the Services.
